This seems similar to Double Airports in Alaska but I can replicate this by installing the current Navigraph AIRAC 2604 rev.1 package again. When installed I see multiple versions of some airports – the worst I found was 3 versions of Nanwalek but there are duplicates of others. I removed AIRAC 2604 rev.1 and everything went back to normal. I reinstalled it and things went back to being duplicated – and it is repeatable for me.
Here is the MSFS 2024 thread – I think started by same user as above post here but a different user reported that reinstalling the navigraph AIRAC resolved their issue but that didn’t resolve it for me (and I’ve switched back and forth four times now and cleared the rolling cache). Removing it resolves the issue, but reinstalling it will put the duplicates back.
Made a short video showing results without AIRAC installed and then with it installed. This does NOT mean Navigraph is at fault or incorrect, I understand that, but maybe this helps figure out what is going on.

I have looked into your example, NANWALEK, and you are right, but there is a but 
ASOBO/MS uses the airport code KEB (the IATA code) for Nanwalek instead of PAAW. You see this too: when you look in the MSFS Flight Planner, you see PAAW, but you can´t use it because it´s not “included” in MSFS for this airport code. You can only use it with KEB and not via PAAW.
Here from the MSFS Flightplanner - you don´t find KEB, you find PAAW, which also we offer:
That is exactly the issue with duplicate airports. We are currently comparing only the airport codes, and when they differ, we “create” the airport, as in this case.
The PKEB was a bug and is fixed from the next AIRAC on. We have improved the comparison from 2605 on, but that does not mean you won´t still find any duplicate airports. We have only a limited number of possibilities to check this. Please report it again if you see any duplicate airports starting with the next cycle.
